Library Card Application Form

Please fill out this form to apply for a new library card.  Your application will be processed within three business days. You will receive a call when your card is ready.

When you pick up your card you must provide a valid photo I.D. with your current address. The following types of photo I.D. are acceptable if they are not expired:

  • government agency issued identification card
  • passports from any country
  • any state driver's license
  • any state or city identification card
  • school identification card

Children under the age of 4 are not eligible to receive an individual library card.

Applicants under the age of 18 are required to have a parent/guardian fill out the parent/guardian agreement.

If you are renewing your card, you must renew in person. Library cards expire every 3 years.  All card applications are processed within three business days. If you have any questions please contact the Circulation Desk 856-767-7727 or via email to beth@njlibraries.org

In order to access the multiple on-line services the library has to offer remotely, you must create a password that will be associated with your online account. Minimum of 5 characters. Please keep a record of your password. The library is able to reset passwords, not retrieve them.
If you sign up for email or text message alerts, you will also receive courtesy notices before items are due, your library card expires, and your card is blocked due to an unresolved missing item.
The Library will use the personal information you provide for official purposes only. We do not sell our list of account holders to other organizations or groups.
By submitting this application, you are agreeing to be responsible for the following: all materials borrowed with your card, paying all fines and fees associated with your card, reporting the loss, theft, or abuse of your card immediately (this includes your responsibility for paying all fines and fees and any items checked out on your card prior to being reported lost or stolen), and to report any changes in your account information. You are also stating that this is your only library card from the Waterford Township Public Library. Providing false information on this application is a felony and is punishable by New Jersey State Law - N.J.S.A. 2C:21-4a.
As a parent or legal guardian of this borrower under the age of eighteen, I agree to be responsible for this borrower's selection and use of library materials, to pay all fines and fees associated with this card, to report the loss, theft, or abuse of this card immediately, and to report any changes in this account's information. I understand that I am responsible for all fines and any items checked out on this card prior to being reported as lost or stolen. Applicants under the age of eighteen will be required to complete a new library card application upon turning eighteen. Providing false information in this application is a felony and is punishable by New Jersey State Law - N.J.S.A. 2C:21-4a.
